DESCRIPTION:On Tuesday\, November 28\, 2023 (10:00–1:00 PT; 1:00-4:00 ET)\, Education Development Center (EDC) will host the National Dementia Care Collaborative (NDCC) Autumn Summit\, The CMS GUIDE Model: Choices for Implementing Evidence-Based Dementia Care. This event is made possible by the generous support The John A. Hartford Foundation\, from a grant through UCLA\, and Summit Co-Sponsor\, Alzheimer’s Association. \nDuring the NDCC Autumn Summit\, presenters will discuss the Guiding an Improved Dementia Experience (GUIDE) Model from the Centers for Medicare & Medicaid Services. The GUIDE Model seeks to support people living with dementia and their family caregivers and care partners\, to promote comprehensive\, high-quality dementia care\, and to address health disparities in dementia. \nAutumn Summit participants will: \n\nLearn how available evidence-based\, comprehensive dementia care models can be implemented to help meet GUIDE Model requirements.\nLearn about the GUIDE Model application process with insights from the Center for Medicare and Medicaid Innovation (CMMI)\nConsider which evidence-based models align with different settings and needs.\nHear directly from evidence-based model developers and other national leaders\, including opportunities to ask questions in breakout groups.\n\nWe hope to see you on November 28! \nTo register\, please click here to complete the registration form.
