A data driven approach resulted in a successful collaboration with the County and expansion of H2H to other difficult to discharge complex and vulnerable populations. DESCRIPTION:National Institute of Aging IMPACT Collaboratory Announces Request for Applications \nJanuary 30\, 2023 – The National Institute on Aging (NIA) Imbedded Pragmatic Alzheimer’s\nDisease (AD) and AD-Related Dementias (AD/ADRD) Clinical Trials (IMPACT) Collaboratory is\nhappy to announce a Request for Applications for the Pilot Grants Program Cycle 5A.\nIn this cycle\, the IMPACT Collaboratory will fund up to five 1-year awards for pilot pragmatic\nclinical trials (ePCTs) that test non-pharmacological interventions embedded in health care\nsystem(s) for people living with Alzheimer’s Disease (AD) and AD-related dementias (ADRD)\nand their care partners. Pilot Studies will be funded for up to $175\,000 in direct costs for a\nmaximum of 12-months. DESCRIPTION:This Faith Community Nurses Network (FCNN) Symposium is geared toward a faith community nurse audience. Individuals outside the core audience are welcome\, but space may be limited on a first-come\, first-serve basis. \nThursday\, January 19 from 9:15 am to 1:00 pm (Central Time) via Zoom \n\nThe prevalence of dementia\, including Alzheimer’s Disease\, is rising as the older population grows. Early detection has become a national priority\, with the goals of preventing avoidable complications and optimizing overall health and wellbeing for affected individuals and their families. FCNs could play an important role in early detection\, assuring referral to appropriate resources\, and advocating for individuals and families. This symposium will provide information to enhance FCNs’ knowledge and skills in recognizing cognitive impairment and promoting good care.\n\nFCN Nursing Scope and Standards: Standard 1 – Collects pertinent data and information relative to the health care consumer’s health and/or situation.\n\nLearning Objectives:\n1. Explain the importance of early detection and education in early cognitive changes and the role of FCNs in this process.\n2. DESCRIPTION:*Presented by National Alzheimer’s and Dementia Resource Center* \nWe invite you to join the National Alzheimer’s and Dementia Resource Center on December 13\, 2022 from 2-3 p.m. ET as we host the webinar\, Dementia Equity Programs in the Hispanic and LGBTQI+ Communities.  A description of the webinar and registration information follows. \nIn this webinar\, participants will learn about dementia programs that focus on serving the needs of the Hispanic and LGBTQI+ communities\, which historically have not received equitable care and support. \nSaul Juarez Aguilar will present information on various challenges to implementing new dementia initiatives in the Hispanic community and how collaboration continues to be an essential and powerful tool for promotion and sustainability. Understanding these challenges and efforts to serve the Hispanic community is essential as approximately 13% of Hispanics 65 or older have Alzheimer’s or another dementia\, and 85% of Hispanics say it is essential for Alzheimer’s and dementia care providers to understand their ethnic or racial background and experiences\, according to the Alzheimer’s Association. Well-established health initiatives educate the Hispanic community about dementia; however\, failing to adapt or understand the Hispanic culture could lead to increased health disparities among Hispanic community members living with dementia. \nSarah Bahnson will discuss some of the challenges experienced by LGBTQI+ people living with dementia and ways to improve services to this community and will present key features of the Visibility Matters program.  Learning about these topics is important as LGBTQI+ people have a unique history that informs contemporary challenges and barriers to accessing aging care services\, including dementia care.
